Why processed meat and smoking sit in the same cancer group

IARC put processed meat in Group 1 alongside tobacco and asbestos, and the comparison has been repeated ever since. The agency answered it directly at the time: the groups rank how strong the evidence is that something can cause cancer, not how dangerous it is — and IARC renamed the whole series in 2019 to say so.

In October 2015 the International Agency for Research on Cancer put processed meat in Group 1, its top category. Tobacco smoking is in Group 1. So is asbestos. The headlines wrote themselves, and a decade later the comparison is still repeated whenever bacon comes up.

The agency answered the question directly at the time, in its own published Q&A, and the answer is worth reading in full before anything else.

What IARC said when asked

"No, processed meat has been classified in the same category as causes of cancer such as tobacco smoking and asbestos (IARC Group 1, carcinogenic to humans), but this does NOT mean that they are all equally dangerous. The IARC classifications describe the strength of the scientific evidence about an agent being a cause of cancer, rather than assessing the level of risk."

The capitals are theirs.

Hazard and risk are different questions

The distinction the answer rests on is set out in the Monographs Preamble, the document that governs how every classification is made:

"A cancer hazard is an agent that is capable of causing cancer, whereas a cancer risk is an estimate of the probability that cancer will occur given some level of exposure to a cancer hazard. The Monographs assess the strength of evidence that an agent is a cancer hazard. The distinction between hazard and risk is fundamental."

A Group 1 listing therefore answers one question — how confident are we that this can cause cancer in humans — and says nothing about how much cancer, at what dose, in how many people. Those are the questions a risk assessment asks, and IARC does not do risk assessment.

⚠️ This is not a caveat the agency added under pressure after 2015. It renamed the entire series over it. The Monographs were once "on the Evaluation of Carcinogenic Risks to Humans"; they are now "on the Identification of Carcinogenic Hazards to Humans", and the Preamble records the change: "in 2019, 'evaluation of carcinogenic risks' became 'identification of carcinogenic hazards', in line with the objective of the programme."

What the groups mean

Group 1 is "The agent is carcinogenic to humans", and it "applies whenever there is sufficient evidence of carcinogenicity in humans". Group 2A is "probably", Group 2B is "possibly", and both turn on how many of three evidence tests are met rather than on how dangerous the thing is.

Group 3 is the one that gets misread in the other direction. It means "not classifiable", and the Preamble is blunt about what that is not:

"An evaluation in Group 3 is not a determination of non-carcinogenicity or overall safety."

⚠️ There is no Group 4 any more. It existed for a single agent, caprolactam, and the 2019 revision removed the category; the classification list annotates that entry "Moved to Group 3 following 2019 update to the IARC Monographs Preamble". An advisory group had noted the public misunderstanding a "probably not carcinogenic" label invited. Older PDFs of the list still circulating do show Group 4, because they predate the change.

What the numbers look like when you ask the risk question instead

IARC did publish a magnitude alongside the classification:

"The experts concluded that each 50 gram portion of processed meat eaten daily increases the risk of colorectal cancer by 18%."

⚠️ That is an 18% increase in an individual's existing risk, not an 18% chance of getting cancer, and it comes from an analysis of ten studies. The head of the Monographs programme said so in the same release: "For an individual, the risk of developing colorectal cancer because of their consumption of processed meat remains small, but this risk increases with the amount of meat consumed."

For scale, IARC's Q&A quoted attributed-death estimates from the Global Burden of Disease Project — an independent research group, not IARC — putting about 34,000 cancer deaths a year worldwide down to diets high in processed meat, against about a million a year from tobacco smoking, 600,000 from alcohol and more than 200,000 from air pollution.

⚠️ Those are 2015-vintage figures from that project, quoted by IARC to make exactly this contrast. They are not IARC's own estimates and they are not current epidemiology; the project has been re-run several times since.

Why comparing within a group is the mistake

IARC's general Q&A has a heading that is the whole argument: why two agents in the same group should not be compared.

"Active smoking carries a much higher risk of lung cancer than do second-hand smoke or air pollution, although all three are classified in Group 1."

Three things vary independently of the group, in their account: how many people are exposed and to how much, the magnitude of risk at that exposure, and how many cancers result. Air pollution reaches nearly everyone; 1,2-dichloropropane, also Group 1, reaches very few. Tobacco causes common cancers; that chemical causes a rare bile-duct one.

What this does not mean

It does not mean the processed meat finding is weak. Group 1 is the strongest evidence category there is, and the classification says the causal link is established rather than suspected.

Nor does it mean there is a safe amount. IARC was asked and declined to give one: the data available "did not permit a conclusion about whether a safe level exists". The honest position is that the risk rises with the amount, and that nobody drew a line.

What to do with it

When a story says something has been "classified as a carcinogen", the useful follow-up is not which group. It is how large the effect is at realistic exposure, and how many people are exposed — the two things the group deliberately does not encode.

Both were published for processed meat on the same day as the classification, by the same agency, and almost none of the coverage used them.

Where this comes from

The hazard-and-risk passage and the group definitions are from the IARC Monographs Preamble as amended in January 2019, section A.2 and section B.6(d). The bacon answer and the safe-level answer are from IARC's Q&A on the carcinogenicity of red meat and processed meat, published with Volume 114 in 2015. The 18% figure and the accompanying quotation are from IARC Press Release N° 240, 26 October 2015. The comparison passage is from IARC's general Monographs Q&A of 10 December 2019. Group memberships are from the Commission's classification list: processed meat Volume 114, tobacco smoking Volumes 83 and 100E, solar radiation 55 and 100D, alcoholic beverages 44, 96 and 100E.

⚠️ The counts are from the Preamble's own text: "Group 4" occurs zero times, against "Group 3" three times, "Group 2A" five and "Group 1" four — the category is absent from a document that plainly discusses the others. The classification list was read in its post-2019 revision, because the widely-circulated alphabetical edition from 2018 still shows caprolactam in Group 4 and would give the opposite answer.
